The current understanding of the molecular mechanisms of these diseases has led to the introduction of targeted therapies which are able to selectively block the key mediators of the inflammation. In particular, research has primarily focused on the development of biologic drugs, but recently, small molecule drugs have been licensed for its use in ulcerative colitis. Janus kinase inhibitors (JAKi) represent the most important family of these drugs, as their peculiar mechanism of action is able to target simultaneously the effects of multiple cytokines and, as a consequence, multiple inflammatory pathways involved in the pathogenesis of the disease are effectively blocked. Here, we discuss the current evidence about the mechanism of action with an up to date review of the efficacy and safety of small molecule drugs in inflammatory bowel disease.
