Clinical risk management constitutes a central element in the healthcare systems in relation to the reverberation that it establishes, and as regards the optimization of clinical outcomes for the patient. The starting point for a right clinical risk management is represented by the identification of non-conforming results. The aim of the study is to carry out a systematic analysis of all data received in the first three years of adoption of a reporting system, revealing the strengths and weaknesses. The results emerged showed an increasing trend in the number of total records. Notably, 86.0% of the records came from the medical category. Moreover, 41.0% of the records reported the possible preventive measures that could have averted the event and in 30% of the reports are hints to be put in place to avoid the repetition of the events. The second experimental phase is categorizing the events reported. Implementing the reporting system, it would guarantee a virtuous cycle of learning, training and reallocation of resources. By sensitizing health workers to a correct use of the incident reporting system, it could become a virtuous error learning system. All this would lead to a reduction in litigation and an implementation of the therapeutic doctor-patient alliance.Bacteriophages (phages) are considered as one of the most promising antibiotic alternatives in combatting bacterial infectious diseases. However, one concern of employing phage application is the emergence of bacteriophage-insensitive mutants (BIMs). Here, we isolated six BIMs from E. coli B in the presence of phage T4 and characterized them using genomic and phenotypic methods. Of all six BIMs, a six-amino acid deletion in glucosyltransferase WaaG likely conferred phage resistance by deactivating the addition of T4 receptor glucose to the lipopolysaccharide (LPS). This finding was further supported by the impaired phage adsorption to BIMs and glycosyl composition analysis which quantitatively confirmed the absence of glucose in the LPS of BIMs. Since LPSs actively maintain outer membrane (OM) permeability, phage-induced truncations of LPSs destabilized the OM and sensitized BIMs to various substrates, especially to the food-grade surfactant sodium dodecyl sulfate (SDS). This hypersensitivity to SDS was exploited to design a T4-SDS combination which successfully prevented the generation of BIMs and eliminated the inoculated bacteria. Collectively, phage-driven modifications of LPSs immunized BIMs from T4 predation but increased their susceptibilities as a fitness cost. This study covers the validation of a centrifugal liquid sedimentation method for accurate measurement of the Stokes diameter of silica particles with a near-spherical shape and dimensions in the nanometer and sub-nanometer scale range. The validated method provided unbiased results in the range of 50 nm to 200 nm, with a lower limit of detection of ≤20 nm. The relative standard uncertainties for precision, quantified in terms of repeatability and day-to-day variation, ranged from 0.2% to 1.0% and from less then 0.1% to 0.5%, respectively. The standard uncertainty for trueness was assessed at 4.6%. Within its working range, the method was found robust with respect to the type of cuvette, light factor, operator, and for defining the meniscus of the sample suspension. The aim was to determine the effects of precipitation heat treatment on the mechanical properties of elements additively manufactured using three different process parameters. Some samples were additionally obtained using hot isostatic pressing (HIP), while some were treated using two different types of heat treatment and a combination of those two processes. From each manufactured sample, a part of the material was taken for structural analysis including residual stress analysis and microstructural investigations. In the second part of the research, the mechanical properties were studied to define the scleronomic hardness of the samples. Finally, tensile tests were conducted using a digital image correlation (DIC) test and fracture analysis. The treated samples were found to be significantly elongated, thus indicating the advantages of using precipitation heat treatment.
