These genetic associations provide a powerful opportunity for understanding the etiology of human disease. HLA class I associations are often interpreted in the light of 'protective' or 'detrimental' CD8+ T cell responses which are restricted by the host HLA class I allotype. However, given the diverse receptors which are bound by HLA class I molecules, alternative interpretations are possible. As well as binding T cell receptors on CD8+ T cells, HLA class I molecules are important ligands for inhibitory and activating killer immunoglobulin-like receptors (KIRs) which are found on natural killer cells and some T cells; for the CD94NKG2 family of receptors also expressed mainly by NK cells and for leukocyte immunoglobulin-like receptors (LILRs) on myeloid cells. The aim of this study is to develop an immunogenetic approach for identifying and quantifying the relative contribution of different receptor-ligand interactions to a given HLA class I disease association and then to use this approach to investigate the immune interactions underlying HLA class I disease associations in three viral infections Human T cell Leukemia Virus type 1, Human Immunodeficiency Virus type 1 and Hepatitis C Virus as well as in the inflammatory condition Crohn's disease.BACKGROUND Insertion of a ventriculoperitoneal shunt to treat hydrocephalus is one of the most common neurosurgical procedures worldwide. Shunt infection affects up to 15% of patients, resulting in long hospital stays, multiple surgeries and reduced cognition and quality of life. OBJECTIVES The aim of this trial was to determine whether or not antibiotic-impregnated ventriculoperitoneal shunts (hereafter referred to as antibiotic shunts) (e.g. impregnated with rifampicin and clindamycin) or silver-impregnated ventriculoperitoneal shunts (hereafter referred to as silver shunts) reduce infection compared with standard ventriculoperitoneal shunts (hereafter referred to as standard shunts). DESIGN This was a three-arm, superiority, multicentre, parallel-group randomised controlled trial. Patients and a central primary outcome review panel, but not surgeons or operating staff, were blinded to the type of ventriculoperitoneal shunt inserted. SETTING The trial was set in 21 neurosurgical wards across the UK and the curred in 6% of participants receiving standard shunts, 5.9% of participants receiving silver-impregnated shunts and 2.2% of participants receiving antibiotic-impregnated shunts. This study has demonstrated a major reduction in shunt infections in new shunts when using antibiotic-impregnated shunts compared with standard or silver-impregnated shunts. A health economic analysis has indicated that antibiotic-impregnated shunts are cost-saving.in English, Spanish This paper analyzes the media coverage of the advance of the Covid-19 pandemic in Argentina and the measure of preventive and obligatory social isolation decreed by the national government. It highlights problematic trends and good practices according to a theoretical and normative approach, and links them to the historical characteristics of the media system in the country. It uses a theoretical-methodological perspective that conceives communication as a fundamental human right, essential in a democratic society, and the mass media as a fundamental vehicle for its exercise, and is supported by the recommendations made by international agencies, civil society organizations and national institutions. The argument is that mass media in Argentina have the historical opportunity to show the relevance of their contribution not only to face this situation, but also to come out of it as a better and more just society.in English, Spanish El impacto que ha originado la enfermedad por coronavirus 2019 (COVID-19) en diferentes partes del mundo, alcanza en la actualidad 597, 072 personas contagiadas y 27,364 fallecidas según los últimos reportes. En ese contexto, en nuestro país, una zona de relevancia epidemiológica es la amazonia peruana, debido a la distribución de enfermedades endémicas como las enfermedades metaxénicas (Dengue, Malaria entre otras), en donde el problema se incrementa debido a que la infección por COVID-19 puede llevar a falsos positivos en las pruebas de cribado para Dengue. Conllevando de esa forma a un retraso en el diagnóstico de la infección por COVID-19 y una mayor diseminación del virus, debido a que en la mayor parte de los casos de Dengue no se presentan signos de alarma y el tratamiento es ambulatorio. Este artículo busca emitir una opinión sobre la necesidad del abordaje de casos de coinfección entre Dengue y Covid-19 en zonas endémicas.in English, Spanish Introducción la enfermedad celiaca es una afección médica que se caracteriza por sus variadas formas de presentación clínica y por su asociación a otras patologías autoinmunes. Objetivo describir caso de paciente que se presentó a consulta por alopecía areata. Metodología estudio observacional, descriptivo, retrospectivo de varón obeso de 24 años Resultados entre los estudios de rutina se detecta serología positiva para enfermedad celiaca que se confirma con endoscopía digestiva. Conclusiones la alopecía areata de este paciente permitió detectar otra enfermedad autoinmune.in English, Spanish Introducción El Neurofibroma (NF) es un tumor benigno que se origina a partir de las células neuronales, pudiendo presentarse en forma solitaria o múltiple. Su manifestación dentro de la cavidad bucal es poco frecuente, con un rango etario amplio de aparición. Se presenta de forma nodular de base sésil, de crecimiento lento y tamaño variable. Objetivo El propósito de este trabajo es presentar un caso clínico de un neurofibroma solitario, de manifestación poco frecuente, con asiento en la cavidad bucal. Métodología Metodología Se analiza el caso de un paciente de sexo masculino de 86 años de edad, que concurre a la consulta con una asimetría facial del lado derecho sin sintomatología dolorosa. A la inspección se observa una lesión nodular de base sésil, móvil, de aproximadamente 5cm de largo por 3cm de ancho. Como tratamiento se decide realizar la extirpación quirúrgica completa de la masa tumoral.
