The development of risk assessment models from ML using data from cardiac CT could aid in the stratification of patients who undergo cardiac CT in different risk classes and better tailor their treatment to individual conditions. While ML is a powerful tool with great potential, applications in the field of cardiac CT are still expanding, and not yet routinely available in clinical practice due to the need for extensive validation. Nevertheless, ML is expected to have a big impact on cardiac CT in the near future.PURPOSE To describe a surgical approach using intraoperative optical coherence tomography and endoscopy for successful Argus II retinal prosthesis system removal. METHODS Retrospective review of a patient undergoing Argus II explantation 8 months after initial implantation. RESULTS Successful explantation of the Argus II device was performed in this patient. CONCLUSION Explantation of the Argus II device can be a difficult surgery, and intraoperative optical coherence tomography and endoscopy can be used to help avoid potential complications.BACKGROUND/PURPOSE Poretti-Boltshauser syndrome is a rare, nonprogressive neurologic syndrome with characteristic cerebellar cysts on neuroimaging due to mutations in LAMA1. The ophthalmic findings in Poretti-Boltshauser syndrome are not well described. Here, we report the ophthalmic findings from multimodal imaging and electrophysiology of a patient with genetically confirmed Poretti-Boltshauser syndrome. METHODS A 3-year-old boy with confirmed mutations in LAMA1 underwent examination under anesthesia with electroretinography and multimodal imaging including fundus photography, fluorescein angiography, optical coherence tomography, and optical coherence tomography angiography. RESULTS Dilated fundus examination was notable for retinal vascular anomalies, including a large area of nonperfusion in the temporal macula with corresponding retinal thinning on optical coherence tomography. There was an absence of a distinct foveal avascular zone and decreased density of both the superficial and deep vascular plexuses in the macula on optical coherence tomography angiography. There was diffuse loss of choriocapillaris architecture and decreased choroidal thickness. CONCLUSION Patients with Poretti-Boltshauser syndrome may possess chorioretinal thinning and retinal vascular abnormalities appreciable on examination and multimodal imaging. These findings suggest a role for LAMA1 in retinal and choroidal vascular development.Accurate BP measurements are vital for determining appropriate medication and treatment regimens. Using a case study as an example, this article discusses the diagnosis of a patient with NMS, the use of ECT as a treatment for NMS, and the importance of nursing care for these patients.BACKGROUND The objective was to determine if women with history of radiation prior to mastectomy experience similar risk for postoperative complications and patient reported outcomes to women undergoing post mastectomy radiation therapy in the setting of immediate implant reconstruction. METHODS The cohort included patients undergoing immediate implant reconstruction at 11 centers across North America. Women were categorized into three groups prior breast conservation therapy, post mastectomy radiation therapy, and no radiation therapy. Mixed effect logistic regressions were used to analyze the effects of radiation status on complication rates and patient-reported outcomes. https://www.selleckchem.com/products/trastuzumab-deruxtecan.html RESULTS 1594 patients were analyzed, including 84 women with prior breast conservation therapy, 329 women who underwent post mastectomy radiation therapy, and 1,181 women with no history of radiation. Compared with post-mastectomy radiation, breast conservation was associated with lower rates of all complications and major complications (OR 0.65, 95% CI 0.37-1.14, p=0.13; OR 0.61, 95% CI 0.33-1.13, p=0.12). These differences were not statistically significant. Rates of reconstructive failure between the two cohorts were comparable. Prior to reconstruction, satisfaction with breast was lowest for women with prior breast conservation therapy (p less then .001). At two years post-operatively, satisfaction with breast was lower for women with post mastectomy radiation, compared with breast conservation patients (p=0.007). CONCLUSIONS Higher postoperative complication rates were observed in women exposed to radiation regardless of timing. Though women with prior breast conservation experienced greater satisfaction with their breasts and fewer complications when compared to women undergoing post mastectomy radiation, there was a similar risk for reconstruction failure.
