BACKGROUND Activation of the mTOR pathway is observed in neurofibromatosis type 1 (NF1) associated low grade gliomas (LGGs), but agents that inhibit this pathway, including mTOR inhibitors, have not been studied in this population. We evaluate the efficacy of the orally administered mTOR inhibitor everolimus for radiographically-progressive NF1-associated pediatric LGGs. METHODS Children with radiologic-progressive, NF1-associated LGG and prior treatment with a carboplatin-containing chemotherapy were prospectively enrolled on this phase II clinical trial to receive daily everolimus. Whole blood was analyzed for everolimus and markers of PI3K/mTOR pathway inhibition. Serial MRIs were obtained during treatment. The primary endpoint was progression-free survival at 48 weeks. RESULTS Twenty-three participants (median age 9.4y; range 3.2-21.6y) were enrolled. It is performed in order to narrow the vaginal opening to ensure vaginal bleeding with a penetration. There are various kinds of techniques with different success rates. OBJECTIVES We report a new hymenal reconstruction technique with vestibulo-introital tightening with the results of 145 procedures. METHODS The new technique includes diamond shaped incision to the vestibulum with the base in the posterior midline and superior corner 2-3 cm higher above the hymen. The angles are accommodated according to the degree of tightening and the submucosal layer is closed from the apex downwards involving the vaginal mucosa. RESULTS The satisfaction rate of the patients was 99.3 %. No adverse events were observed. CONCLUSIONS Compared with previous techniques described, this hymenal reconstruction technique is an alternative with the advantage of low risk of loosening because the tension on hymen alone is decreased. Doses were measured on a phantom for specific C-arm projections typically used during CA/PCI procedures. In order to estimate the cumulative eye lens doses, conversion coefficients were then applied to the dose records of interventional cardiologists collected in the database of dosimetry service between the years 1995 and 2009. The Hp(10) to Hp(3) conversion coefficients are 0.29 (CV = 34%) and 0.17 (CV = 42%) for left and right eye lens, respectively. However, they can vary from one laboratory to another depending on working technique. From among 61 interventional cardiologists, none exceeded the threshold dose of 0.5 Gy for eye lens opacities. METHODS We performed a nationwide population-based cohort study of all Danish patients with an ECI diagnosed during 1997-2016 (n=1745 patients) and a comparison cohort from the general population individually matched on date of birth and sex (n=17,450). Outcomes were categorized into mortality and risk of cancer and likely measures of neurological sequelae including neuropsychiatric morbidities, educational landmarks, use of hospital services, employment, receipt of disability pension, income, number of sick leave days and nursing home residency. RESULTS Mortality in the first year was increased among patients with ECI (mortality rate ratio=10.0, 95% CI 4.17 - 24.1), but thereafter mortality was not increased (mortality rate ratio=0.94, 95% CI 0.47 - 1.86). The long-term outcomes for patients with ECI were not inferior to those of the comparison cohort what concerns risk of cancer, epilepsy, mental and behavioral disorders, dementia, depression, school start, school marks, high school education, use of hospital services, employment, receipt of disability pension, income, days of sick leave, or nursing home residency. CONCLUSION A diagnosis of an ECI had no substantial impact on long-term survival, health, or social/educational functioning. © The Author(s) 2020.
