It is a platform that collects and shares clean trial data and ratings using PRECIS-2 permitting multiple and varied meta-epidemiological studies to evaluate the effect of treatment on trials with different levels of pragmatism, as well as other design features. Background Pragmatic studies are increasingly recognized as providing real-world evidence for clinical decision making. However, the use of the term "pragmatic" is inconsistent and its definition and evaluation requires clarification. The purpose of pragmatic trials is to guide clinical practice and policy decisions, rather than to prove the validity of a clinical or physiological hypothesis. A pragmatic trial should aim to be as close as is possible to real-world clinical practices, including recruiting participants, setting, design, delivery and execution of interventions, determination and analysis outcomes, and primary analyses. This is a major distinction between explanatory trials as defined by Schwartz & Lellouch1, which are designed to test a hypothesis in a more thorough way. Studies that are truly pragmatic should avoid attempting to blind participants or healthcare professionals as this could lead to distortions in estimates of the effects of treatment. The pragmatic trials also include patients from different healthcare settings to ensure that their outcomes can be compared to the real world. Additionally, clinical trials should be focused on outcomes that matter to patients, such as quality of life and functional recovery. is especially important in trials that involve the use of invasive procedures or potential for serious adverse events.
