INTRODUCTION Glaucoma is a degenerative disease in the optic nerve with associated visual field defects (VFD). Trabeculectomy is the most common glaucoma surgery. Surgery is indicated if glaucomatous optic neuropathy progresses despite tolerated medical treatment or in patients with severe VFD. The purpose of this paper is to describe the severity of visual field damage in patients undergoing their first trabeculectomy in Iceland. METHODS A retrospective review of medical records of all patients with open angle glaucoma that underwent first trabeculectomy at Landspítali University Hospital, from June 2013 to March 2016. Visual fields were examined by Octopus automated perimetry and the severity of glaucoma damage was staged according to the mean defect (MD). RESULTS 86 eyes were included in the study, mean age 75 ± 11 years, 57% men. Patients used on average three IOP lowering medications. Mean MD at referral to surgery was 13.4 ± 7.7dB (min 0.8dB, max 26.2 dB), 21% had early glaucomatous damage (MD 12). CONCLUSION VFD at referral to surgery varied from mild VFD to severe damage. Like clinical guidelines recommend, treatment seems to be individualized and the most common indication for surgery was increased VFD despite medical treatment. Mean MD at referral to surgery was high compared to other studies. Eyes with severe VFD had on average lower IOP and thinner cornea. This might indicate that great emphasis is placed on high IOP and perhaps too little emphasis on VFD and cornea thickness.INTRODUCTION Five-year survival of people diagnosed with cancer has increased in past decades. Knowledge of the long-term effects of cancer treatment on fitness, body composition and health related quality of life (HR-QoL) is important for cancer patients. The study aimed to evaluate body composition, HR-QoL and personality D of people post cancer treatment in the last 10 years; and to observe whether fitness was correlated with body composition and HR-QoL amongst them. MATERIAL AND METHODS Eighty participants 25 - 77 y/o of both genders in remission from cancer answered questionnaires on HR-QoL (SF-36v2 and EQ-5D-3L), personality D (DS14), general health, lifestyle and cancer treatment. Participants had their blood pressure measured, conducted the 6-minute walk test (6MWT) and had their body composition meas-ured body mass index, body fat percentage, waist and hip circumferences. SPSS was used for statistical analysis. RESULTS Approximately 2/3 of the participants had body composition over recommended values. Over half were overweight or obese. 66.3% had waist circumference over recommended values and 45.0% had waist/hip ratio over recommended values. 6MWT average distance was 634 m +/- 83 m. Significant correlation (p less then 0.05) was found between 6MWT and body composition and most categories of HR-QoL. -Increased fitness correlated with better body composition and higher HR-QoL. Only 13.8% of participants showed personality D. CONCLUSION Cancer survivor's body composition is generally not at its best. It is important that people diagnosed with cancer are informed on the importance of healthy body composition. METHODS A total of 1588 Japanese adults aged ≥60 years without dementia were prospectively followed from 2002 to 2012. Cox proportional hazards models and restricted cubic splines were used to estimate the HRs of tHcy levels on the risk of dementia. RESULTS During the follow-up, 372 subjects developed all-cause dementia; 247 had Alzheimer's disease (AD) and 98 had vascular dementia (VaD). Compared with the lowest tHcy quintile (≤6.4 µmol/L), the multivariable-adjusted HRs (95% CI) of the highest quintile (≥11.5 µmol/L) were 2.28 (1.51-3.43) for all-cause dementia, 1.96 (1.19-3.24) for AD and 2.51 (1.14-5.51) for VaD. In restricted cubic splines, the risk of all-cause dementia steadily increased between approximately 8-15 µmol/L and plateaued thereafter, with a similar non-linear shape observed for AD and VaD (all p for non-linearity ≤0.02). In stratified analyses by the most recognised genetic polymorphism affecting tHcy concentrations (methylenetetrahydrofolate reductase C677T), the positive association of tHcy with all-cause dementia persisted in both non-carriers and carriers of the risk allele, and even tended to be stronger in the former (p for heterogeneity=0.07). CONCLUSION High serum tHcy levels are associated with an elevated risk of dementia, AD and VaD in a non-linear manner, such that an exposure-response association is present only within a relatively high range of tHcy levels. Published by BMJ.OBJECTIVE The Tysabri Observational Programme (TOP), which began >10 years ago, is an open-label, multinational, prospective observational study evaluating the long-term safety and effectiveness of natalizumab in relapsing-remitting multiple sclerosis patients. METHODS These data provide a 10-year interim analysis of safety and effectiveness in TOP. Annualised relapse rates (ARRs) and disability progression/improvement were analysed using the Poisson model and the Kaplan-Meier method, respectively. Analyses included patients on natalizumab and those who discontinued natalizumab but remained in TOP. RESULTS As of November 2017, TOP included 6148 patients. Overall, 829 patients (13.5%) experienced ≥1 serious adverse event (SAE), with infection the most common (4.1%). Fifty-three patients (0.9%) had confirmed progressive multifocal leukoencephalopathy.
