Objective The objective of the study is to identify specific population groups that may benefit from intraoperative motor evoked potentials (MEP) and to assess positive predictive value (PPV) and negative predictive value (NPV) changes during operation by correlating these with postoperative motor outcomes. Methods We retrospectively reviewed 1,043 consecutive patient cases undergoing spine surgery with and without intraoperative monitoring (IOM) at a single institution from January 1, 2016 to December 31, 2017. Demographic and clinical outcome data were collected at multiple time points. An MEP amplitude decrease of 50% or greater was correlated with a motor deficit for this study. Results On multivariate analysis, patients with coronary artery disease and who received IOM were more likely to experience no new deficit (p=0.047) than those who did not receive IOM. Additionally, patients with hyperlipidemia and coronary artery disease (CAD) were less likely than those without to experience no new deficit (p=0.001 and p=0.02, respectively). MEP accounted for 244 cases, of which 15 had alert MEP criteria but no deficit for a PPV of 21.05% at day 1 post-operation. Day 7-30 PPV declined to 14.29%, and by day 90, there was no association. In our descriptive case series, we present three unique cases of complicated COVID-19 infection in patients with hematologic-oncologic risk factors and review the imaging features of their complications. The first patient was a 33-year-old male with sickle cell trait who developed rhabdomyolysis and myonecrosis of the paraspinal muscle in the setting of a physical fitness test; he subsequently developed an abscess at this site, presumably exacerbated by the hypoxemic state of his COVID-19 pneumonia. Our second patient was a 37-year-old male with COVID-19 pneumonia and a history of stage IV Non-Hodgkin's lymphoma in remission who developed spontaneous pneumomediastinum in the absence of positive pressure ventilation. Three-dimensional augmented reality (3D-AR) is an emerging technology that has the potential to assist physicians with spatial reasoning during clinical interventions. The most intriguing applications of 3D-AR involve visualizations of anatomy or surgical plans that appear directly on the patient. However, commercially available 3D-AR devices have spatial localization errors that are too large for many clinical procedures. For this reason, a variety of approaches for improving 3D-AR registration accuracy have been explored. The focus of this review is on the methods, accuracy, and clinical applications of registering 3D-AR devices with the clinical environment. The works cited represent a variety of approaches for registering holograms to patients, including manual registration, computer vision-based registration, and registrations that incorporate external tracking systems. Evaluations of user accuracy when performing clinically relevant tasks suggest that accuracies of approximately 2 mm are feasible. 3D-AR device limitations due to the vergence-accommodation conflict or other factors attributable to the headset hardware add on the order of 1.5 mm of error compared to conventional guidance. Continued improvements to 3D-AR hardware will decrease these sources of error. The Percutaneous laser disc decompression (PLDD) method was first described by Daniel Choy in Australia in 1987. Therefore, in this study, we examined the clinical signs and symptoms of patients with spinal canal stenosis due to disc protrusion after PLDD surgery. In this clinical trial study, 43 patients with spinal canal stenosis due to lumbar disks who referred to Kashani and Zahra Marzieh educational hospitals from 2006 to 2016 were entered the study. The patients were divided into two groups as discogenic canal stenosis (3 females and 9 males) and complex degenerative disorder (canal stenosis due to discogenic and ligamentos) (16 females and 15 males). Patients underwent PLDD surgery and the clinical manifestations such as back and radicular pain, claudication, and complications of the surgery (hematoma, reoperation, and neurological symptoms) in patients were evaluated until one year after the operation. After one year of surgery, the mean of back and radicular pains significantly decreased in both groups (P<0.05). All patients with claudication in the discogenic group improved and 35.5% of patients with complex degenerative disorder were not claudication after one year of surgery. The outcomes of treatment in patients with discogenic canal stenosis were 91.7% excellent, and 8.3% fair and in the complex degenerative disorder group were 64.5% excellent, 19.4% good and 16.1% fair (P=0.16). None of the patients had new neurological symptoms, and 12.9% of the complex degenerative disorder group patients needed reoperation. The PLDD method is a better procedure for discogenic canal stenosis than complex degenerative disorder. Therefore, more studies are required in this field for long time.
