To compare the surgical outcomes of patients with clinical stage I ground-glass opacity (GGO) lung adenocarcinomas with maximum diameters of≤ 2 cm who underwent lobectomy versus limited resection. We retrospectively reviewed cases of clinical stage I GGO lung adenocarcinoma with a diameter≤ 2 cm that were treated via lobectomy or limited resection in our department between January 2011 and September 2018. The clinical characteristics and surgical outcomes were analyzed using a propensity score-matched comparison and a Cox regression model. A total of 552 patients were identified; 128 patients with pure GGO were excluded. Four hundred twenty-four patients met our criteria, including 242 (57.1%) who underwent lobectomy and 182 (42.9%) who underwent limited resection. No perioperative mortality occurred in either group. The overall 5-year survival rate of the entire cohort was 88%. Patients who underwent limited resection tended to have a shorter operation time, smaller blood loss volume, fewer removed nods with solid-dominant tumor.The incorporation of leadership concepts is at the forefront of agendas to advance the quality of patient care and innovation in many healthcare professions. In this Commentary, the authors draw upon evidence from the literature to discuss the theoretical perspectives of leadership and potential value in radiation therapy practice. For the patients to be provided with safe and high-quality treatment, continuous change and improvement are necessary. Leadership has the potential to influence behaviour change for practitioners and core organisational culture. Several leadership styles such as the transformational leadership can be applied to achieve a higher level of patient care and improve the likelihood of enhancing quality and efficiency in healthcare. These are also paramount to the success of radiation therapy practice. Hence, a higher priority should be placed on developing leadership in the Radiation Therapist (RT) workforce and educating new graduates of the potential benefits. The transition to motherhood, although joyous, can be highly stressful, and the availability of professional postpartum support for mothers is often limited. Peer volunteer support programs may offer a viable and cost-effective method to provide community-based support for new mothers. To determine the feasibility of a peer volunteer support program-The Mummy Buddy Program-in which experienced volunteer mothers are paired with, and trained to offer social support to, first-time mothers. Using a single-group non-randomised feasibility trial, a total of 56 experienced mothers participated in the Mummy Buddy training program, which was focused on education and practical exercises relating to the provision of various forms of social support. Experienced mothers ('Mummy Buddies') were subsequently paired with expectant first-time mothers (n=47 pairs), and were encouraged to provide support until 24-weeks postpartum. We conducted a systematic review and meta-analysis of all studies that compared SAPIEN 3 vs Evolut R in patients undergoing TAVR. The primary endpoint of this meta-analysis was 30-day mortality. Secondary outcomes included major of life-threatening bleeding, risk of stroke, need of permanent pacemaker implantation, and risk of moderate to severe paravalvular regurgitation (PVR). We included a total of 9 studies. One study was a randomized clinical trial, five were prospective observational studies and three were retrospective. 30-day mortality rate was similar between SAPIEN 3 and Evolut R (odds ratio (OR) 1.19; 95% confidence interval (CI) 0.72 to 1.93; p=0.47). The risk of major or life-threatening bleeding (OR of 0.83, 95% CI 0.50 to 1.39; p=0.48), and the risk of stroke (OR of 0.82, 95% CI 0.38 to 1.78; p=0.62) were also similar between the two types of valves. Compared to SAPIEN 3, Evolut R was associated with statistically significant risk of permanent pacemaker implantation (OR of 1.40, 95% CI 1.15 to 1.70; p=0.0007), and moderate to severe PVR (OR of 2.56, 95% CI 1.14 to 5.74; p=0.02). However, optical coherence tomography (OCT) assessment of neointimal distribution after EES implantation is lacking. We aimed to assess the longitudinal neointimal distribution pattern after EES implantation using OCT. Data from 3 prospective studies (HEAL-EES, REVER and RESERVOIR), including patients with EES implantation and OCT follow-up study, were merged. Analyzed stents were divided into 3 segments of equal length (distal, medial, proximal). Longitudinal neointimal distribution patterns were compared between the 3 segments using generalized estimating equation. Neointimal thickness (NIT), neointimal area obstruction, and uncovered or malapposed struts were analyzed. In total, 86 patients (92 lesions) were analyzed. Time of OCT follow-up was 9.0±1.5months. NIT was 101.7±65.4μm and neointimal obstruction area was 12.2±7.6%. The number of assessed struts was the same in all three segments. NIT tended to be highetimal distribution in distal, medial and proximal stent segments. Neointimal hyperplasia seemed to be different between the three segments, with a higher burden in the medial stent segment. Whether this reflects a response to local pre-interventional plaque burden centrally covered by the stent should be confirmed in a future study.
