The peripartum period appears to be a potent trigger of obsessive-compulsive disorder in women; however, due to the lack of awareness and routine screening, women with obsessive-compulsive disorder may be at risk of under- or misdiagnosis. Unfortunately, the missed diagnosis or misdiagnosis can have serious consequences including symptom persistence, compromised ability to care for the newborn, poor quality of life and unnecessary reporting of the mother to child protective services. It is argued that future iterations of the DSM consider expanding the scope of the peripartum onset specifier to include obsessive-compulsive disorder to increase its detection, diagnosis and management.Purpose Loss of appetite is a common side effect of cancer and cancer treatments resulting in risk of malnutrition and cancer cachexia. This review aimed to systematically determine nutrition interventions that improve appetite and nutrition-related outcomes of adults with cancer undergoing cancer treatments, and to identify appetite assessment tools used to measure appetite. Methods Inclusion criteria included randomised controlled trials of adults with cancer undergoing chemotherapy, radiotherapy or immunotherapy treatments, nutrition interventions and appetite assessed by an appetite assessment tool or quality of life tool. The search strategy was applied to four databases and two researchers systematically assessed for eligibility. Following data extraction, quality of the included library was assessed using the Quality Criteria Checklist Primary Research. A narrative synthesis of results was undertaken. Results After title/abstract screening, 24 full texts were assessed for eligibility; five trials of n = 472 participants were included in the final library. Nutrition interventions that improved appetite were oral nutrition supplements, fish oil supplements and dietary counselling. Appetite was assessed via visual analogue scales (n = 1) and EORTC QLQ C30 questionnaire (n = 4). Quality was assessed as neutral in 2 studies and positive in 3 studies. Conclusion The use of oral nutrition supplements and dietary counselling and increases in EPA from fish oil supplementation improved the appetite and nutrition outcomes of patients with cancer undergoing cancer treatments. Validated assessment tools in the oncology setting are needed to determine which nutrition interventions positively influence appetite outcomes.Purpose This study examined associations between fertility consultation (FC) and multiple dimensions of reproductive concerns among young adult (YA) male cancer survivors. Methods One hundred and seventy YA male cancer survivors (age 18 to 35) across the USA completed an online survey between 2016 and 2018. We used log binomial regression to examine associations between FC and high reproductive concerns across multiple domains. Results In multivariate analyses adjusting for desire for children, FC was associated with higher likelihood of having at least one high reproductive concern (relative risk [RR] 1.4, 95% CI 1.2-1.7). Across subscales, FC was associated with greater likelihood of having high concerns about fertility potential (RR 1.7, 95% CI 1.0-3.0), achieving pregnancy (RR 3.5, 95% CI 1.3-9.5), their (potential) child's health (RR 1.5, 95% CI 1.1-2.2), and disclosing infertility to a partner (RR 2.7, 95% CI 1.8-4.1). In contrast, associations were not observed between FC and likelihood of high concerns about personal health (RR 1.4, 95% CI 0.5-3.8) or acceptance of infertility (RR 1.8, 95% CI 0.8-3.9). Conclusions YA male cancer survivors who received FC were more likely to have high reproductive concerns than those who did not receive FC. Men who seek out FC after cancer diagnosis may need additional support for their reproductive concerns. Research is needed to identify strategies to alleviate these concerns among this population.Objectives Veno-arterial extracorporeal membrane oxygenation (VA-ECMO) is increasingly being used to treat patients with refractory severe heart failure. Large animal models are developed to help understand physiology and build translational research projects. In order to better understand those experimental models, we conducted a systematic literature review of animal models combining heart failure and VA-ECMO. Studies selection A systematic review was performed using Medline via PubMed, EMBASE, and Web of Science, from January 1996 to January 2019. Animal models combining experimental acute heart failure and ECMO were included. Clinical studies, abstracts, and studies not employing VA-ECMO were excluded. Data extraction Following variables were extracted, relating to four key features (1) study design, (2) animals and their peri-experimental care, (3) heart failure models and characteristics, and (4) ECMO characteristics and management. Results Nineteen models of heart failure and VA-ECMO were included in this review. All were performed in large animals, the majority (n = 13) in pigs. Acute myocardial infarction (n = 11) with left anterior descending coronary ligation (n = 9) was the commonest mean of inducing heart failure. Most models employed peripheral VA-ECMO (n = 14) with limited reporting. Conclusion Among models that combined severe heart failure and VA-ECMO, there is a large heterogeneity in both design and reporting, as well as methods employed for heart failure. There is a need for standardization of reporting and minimum dataset to ensure translational research achieve high-quality standards.The transvalvular pressure gradient (TPG) is commonly estimated using the Bernoulli equation. However, the method is known to be inaccurate. Therefore, an adjusted Bernoulli model for accurate TPG assessment was developed and evaluated. Numerical simulations were used to calculate TPGCFD in patient-specific geometries of aortic stenosis as ground truth. Geometries, aortic valve areas (AVA), and flow rates were derived from computed tomography scans. Simulations were divided in a training data set (135 cases) and a test data set (36 cases). The training data was used to fit an adjusted Bernoulli model as a function of AVA and flow rate. The model-predicted TPGModel was evaluated using the test data set and also compared against the common Bernoulli equation (TPGB). TPGB and TPGModel both correlated well with TPGCFD (r > 0.94), but significantly overestimated it. The average difference between TPGModel and TPGCFD was much lower 3.3 mmHg vs. 17.3 mmHg between TPGB and TPGCFD. Also, the standard error of estimate was lower for the adjusted model SEEModel = 5.
