Although the caregiving experience appears negative, some positive elements of caregiving experience were prominent among siblings. Furthermore, the violence phenomenon among individuals with schizophrenia needs to be addressed as a priority. Implications for practice Understanding the experience, feelings and needs of relatives living with schizophrenia would enable mental health nurses to provide a range of interventions to help reduce caregivers' burden and promote the positive gains from the caregiving experience. The concept of stigma resistance in the Arab world may guide the establishment of stigma reduction programmes.Papilledema is a syndrome of the retina in which retinal optic nerve is inflated by elevation of intracranial pressure. The papilledema abnormalities such as retinal nerve fiber layer (RNFL) opacification may lead to blindness. These abnormalities could be seen through capturing of retinal images by means of fundus camera. This paper presents a deep learning-based automated system that detects and grades the papilledema through U-Net and Dense-Net architectures. The proposed approach has two main stages. First, optic disc and its surrounding area in fundus retinal image are localized and cropped for input to Dense-Net which classifies the optic disc as papilledema or normal. Second, consists of preprocessing of Dense-Net classified papilledema fundus image by Gabor filter. The preprocessed papilledema image is input to U-Net to achieve the segmented vascular network from which the vessel discontinuity index (VDI) and vessel discontinuity index to disc proximity (VDIP) are calculated for grading of papilledema. The VDI and VDIP are standard parameter to check the severity and grading of papilledema. The proposed system is evaluated on 60 papilledema and 40 normal fundus images taken from STARE dataset. The experimental results for classification of papilledema through Dense-Net are much better in terms of sensitivity 98.63%, specificity 97.83%, and accuracy 99.17%. Similarly, the grading results for mild and severe papilledema classification through U-Net are also much better in terms of sensitivity 99.82%, specificity 98.65%, and accuracy 99.89%. In this regard, it is essential to understand the influence of adding solvents on the properties of ILs. In this work, the micro-heterogeneity and H-bonding interactions between a hydroxyl-functionalized IL, [HOEmim][TFSI], and acetonitrile (ACN) were investigated by attenuated total reflection Fourier transform infrared spectroscopy and molecular simulations. All studied IL-ACN mixtures were found to deviate from the ideal mixtures. The degree of deviations reaches the maximum at about x(ACN)=0.7 with the presence of both homogeneous clusters of pure IL/ACN and heterogeneous clusters of IL-ACN. With the addition of ACN to IL, the mixtures undergo the transformation from "ACN solvated in [HOEmim][TFSI]" to "[HOEmim][TFSI] solvated in ACN". It is found that the newly formed H-bonding interactions between the IL and ACN is the main factor that contributes to the red shifts of O-H, C2 -H, C4,5 -H, and Calkyl -H of [HOEmim]+ cation, and the blue shifts of C-D, C≡N of ACN, and C-F, S=O of [TFSI]- anion. These in-depth studies on the mixtures of hydroxyl-functionalized IL and acetonitrile would help to understand the micro-heterogeneity and H-bonding interactions of miscible solutions and shed light on exploring their applications.The management of pemphigus vulgaris (PV) is challenging. This study aimed to evaluate the immunomodulating effects of metformin on PV. The study was conducted in two phases in the first phase, patients received routine first-line treatment (prednisolone plus azathioprine) for 2 months, then in the second phase, metformin was added to this regimen for another 2 months. After addition of metformin to the first-line medications, significant reductions were seen in serum IgG1 (reduced from 534.92 ± 134.83 mg/dL to 481.58 ± 130.46 mg/dL, P less then 0.001), IgG4 (51.83 ± 27.26 mg/dL to 44.50 ± 26.05 mg/dL, P less then 0.001) and interferon-γ (277.99 ± 108.71 pg/mL to 45.05 ± 17.080 pg/mL, P = 0.03) concentrations. The suppressant effect of metformin was greatest on IgG4 (coefficient of variation 1.28), the dominant subclass of IgG involved in PV. Metformin could have immunomodulating effects on PV with controlling effects on steroid complications.
